A 6" service weight cast iron 90 degree elbow, domestic manufacture. A right angle on a 6" drain is a significant restriction, so it is used where the structure genuinely allows nothing else. Where the turn is from vertical to horizontal, a sweep is the correct fitting instead - the cost difference is trivial next to the cost of a main that blocks.
