A Sondex double wall brazed plate heat exchanger. The double wall construction is the reason to specify this over a standard plate unit: the two circuits are separated by two walls with a vented gap between them, so a plate failure leaks visibly to the outside rather than allowing the fluids to mix. That is what makes it acceptable where a heating circuit meets potable water, and it is often a code requirement rather than a preference.
